REMOVAL STEPS

  2. Select your state
    Choose your state from the dropdown (California, Colorado, Connecticut, Delaware, Indiana, Iowa, Kentucky, Maryland, Minnesota, Montana, Nebraska, Nevada, New Hampshire, New Jersey, Oregon, Rhode Island, Tennessee, Texas, Utah, or Virginia).

  3. Select "Deletion Request" as the request type
    The form also offers Access Request, Appeal a Decision, and Request to Correct — choose Deletion Request for full removal.

  4. Fill out the form
    Enter your first name, last name, and email address.
